The Gateway to Community Living portal allows the Alabama Medicaid Agency to receive and manage transitions to community living from an initial referral through the entire first year after transition occurs. This ensures that each individual’s needs are carefully assessed and a plan for safe transition is developed and implemented.
Frequent users of the Gateway System may include nursing facility staff, transition coordinators from home and community-based waiver programs and other agencies or organizations that support individuals interested in transition. Frequent users must establish a user account before receiving access to the Gateway System, and will receive an email confirmation after completing the registration process. Based on the user category assigned, access will be granted to the appropriate system functions. When applicable, users will be prompted to attach required forms and supplemental documents to the online application. Buttons are in place to facilitate browsing, selecting, and uploading the attachments.
